We are providing comments on the proposal to collect four new items as standardized resident assessment data elements beginning with the FY 2027 Skilled Nursing Facility (SNF) Quality Reporting Program (QRP).
AFH applauds CMS’s use of payment policies to drive improvements in health equity. The OPPS rule governs how Medicare reimburses hospitals and outpatient facilities for services, and CMS is increasingly recognizing that factors like socioeconomic status, access to care, education, and environment (SDOH) directly impact patient health outcomes.
Thank you for the opportunity to provide comments on the Fiscal Year (FY) 2025 proposed Hospital Inpatient Prospective Payment Systems (IPPS) for Acute Care Hospitals and the Long-Term Care Hospital (LTCH) Prospective Payment System rule. We are providing comments on several elements of the proposal, including the proposed changes to the severity level designation for the diagnosis codes describing inadequate housing and housing instability, collection of four new items as standardized patient assessment data elements beginning with the FY 2028 LTCH Quality Reporting Program (QRP), and more.

We appreciate ONC’s inclusion of health equity in the four goals of promoting health and wellness, enhancing the delivery and experience of care, accelerating research and innovation, and connecting the health systems with health data. Overall, we encourage ONC to continue to work closely with private organizations like payers, hospitals, health information management companies, community-based organizations (CBOs), and others to collect strategies and best practices in support of each of these goals.

Aligning for Health thanks you for introducing the Housing Alignment and Coordination of Critical and Effective Supportive Health Services (ACCESS) Act (H.R 7647/S2932), which would increase support for millions of low-income Americans on Medicaid by aligning housing and healthcare resources to address the interconnected challenges posed towards people experiencing homelessness, mental health issues, and substance use disorders.
Aligning for Health thanks you for introducing the Community Health worker Access ACT (S. 3892), which aims to expand the services that community health workers (CHWs) may receive Medicare reimbursement for to include preventive services and those that address social determinants of health and would encourage states to implement the option to reimburse for CHW services under Medicaid.
Aligning for Health thanks you for introducing the Addressing Whole Health in Medicare Advantage Act (H.R. 5746), which would expand access to critically important supplemental benefits for a larger number of Medicare Advantage enrollees who need more robust services to effectively manage their health, including enrollees low-income enrollees and individuals with chronic conditions, at high-risk for hospitalizations, or who require intensive care coordination.
